Patient Information

During your initial consultation your oral surgeon will review your health history and perform an examination; after which your treatment and anesthesia options will be fully explained. Dental x-rays are usually necessary to make a complete diagnosis. A second surgical appointment will be made for your extraction(s), implant(s) or dental surgery.  Under special circumstances it may be possible to complete your oral surgery on the same day as your consultation. Please let us know if you desire to do so.

For convenience, you may choose to print the following two forms and complete the requested information prior to your visit. Remember to bring the completed forms with you to your consultation appointment.   

To assist us with your care, please provide the following information at the time of your consultation.


X-rays are a valuable diagnostic tool for most oral surgery procedures. For your convenience we are able to take any necessary x-rays in our office. If your dentist or physician has recent x-rays, we may request that they forward a copy to our office in time for your consultation appointment.

Scheduling an Appointment

We invite you to call to schedule an appointment Monday through Friday between 8:00 am and 5 pm. We typically reserve morning appointments for our surgical patients undergoing anesthesia and afternoons for all other types of appointments. We are sensitive to your busy schedule and will attempt to accommodate your scheduling constraints. If you are experiencing facial pain or find yourself in an oral surgical urgent situation, we will work to appoint you as soon as possible.

We respect that your time is important. It is our goal to stay on time throughout the day. Please help us maintain this goal by arriving a few minutes before your scheduled appointment time. Should you need to reschedule an appointment, the courtesy of 48 hours notice is appreciated.
